Our Practice operates a policy of accepting repeat prescription requests in written form only. You can choose to order your Repeat Prescription in one of three easy ways:

1. Download the Repeat Prescription Form here, print it out and complete it. Once completed please return your form to us either by email, by post or drop it in to us.
   
   
2. Drop into the practice, pick up a Repeat Prescription Form, fill it in and drop it back to us.
   
   
3. Send us your list by post.

Your prescription will be ready to collect in two working days.

Please Note: We can provide patients with a list of their long term medications upon request.
